Sportsmed Reward Card

Sportsmed are partnering selected* sports clubs and schools in the community in giving something back to these organisations.

The Sportsmed Reward Card is a unique card that allows your club or school to receive a rebate from Sportsmed each time you receive treatment at Sportsmed and present your card. Thus the more treatments you have the more your club or school benefit.

Firstly your club must be affiliated with the Sportsmed reward card scheme. A list of the clubs and organisations are highlighted below and is constantly updated. There are 2 ways you can register to receive your card.

If you have received a form requesting a Sportsmed Reward Card then all you need to do is fill this in and post it to us and a card will be sent to you.
You can complete the details in the section below and register for your card online. If you do choose to order your card online then you will go in a draw to win an Asics gift pack to the value of $500.

Before your card can be mailed to you we will confirmation eligibility with your club or school.
